You will oversee the review and monitoring of patient safety incidents, using data-driven insights to identify trends and opportunities for improvement through our event reporting system.In this role, you will work closely with leaders and clinical teams to standardize the response to safety events, develop actionable improvement plans, and help drive progress towards our quality objectives. You will be an advocate for a “just culture,” emphasizing the importance of event reporting, conducting thorough reviews, implementing recommendations, and sharing insights to strengthen system-wide practices.This position requires strong collaboration with internal and external stakeholders, as well as strategic involvement in planning, developing, and evaluating organizational processes to ensure seamless integration and continuous improvement in patient safety standards.Required
- Completion of a post-secondary program in the Health Sciences field of study.
- Quality improvement experience in health care field.
- Patient Safety Officer Certification – Preference will be given to candidates with this
certification.
- Familiarity and demonstrated commitment to Just Culture.
- Knowledge of change management, rapid cycle improvement, evaluation methods,
communication.
- Evidence of data literacy.
- Demonstrated effective communication with the healthcare team, patients & families.
- Proven interpersonal and conflict management skills.
- Excellent critical thinking/decision-making knowledge/skills – time management and
prioritization skills.
- Program design, development and presentation skills.
- Exceptional computer/ technology skills – Proficient in Microsoft Word, Outlook, Excel, and
Power Point.
- Demonstrates exceptional interpersonal, verbal and written communication skills including
ability to foster collaborative relationships as required.
- Working knowledge of relevant legislation – Excellent Care for All Act, Quality of Care
Information Protection Act, Public Hospitals Act, Apology Act.DutiesPatient Safety Event Management
- Works with all members of the quality team to manage the patient safety event reporting
system.
- Promote and provide a culture of support for physicians and staff following a patient
safety event.
- Facilitate the use of a patient safety event reporting framework for clinical programs and
others as requested.
- Provide feedback to Directors/Managers regarding occurrences, trends, and high-risk
activities identified via the patient safety event reporting system.
- Daily monitoring of patient safety events, including conducting initial reviews and
triggering alerts to the Manager, Quality and Risk for reporting to Senior Leadership and
hospital insurance provider as per policy when necessary.
- Support the Manager, Quality and Risk to lead physicians and staff through the quality
review process. This could include gathering information, developing a timeline, c
capturing recommendations, and supporting evaluation recommendation efficacy.
- Ensure timely review and follow-up of patient safety events that are deemed not to
require a quality review. Those events that require a quality review include critical
events (level 1 and 2) as well as some moderate harm events (level 3) and their review
will be led by the Manager, Quality and Risk.
- Develop regular patient safety reports, including data analysis and quality review status,
for operational oversight committees and the Board Quality Committee.
- Work in partnership with the Manager, Quality and Risk to review and update the Event
reporting system templates.Patient Safety
- Promote a just culture and a culture of safety and improvement through strong
leadership and ongoing coaching and mentoring at all levels of the organization.
- Work with members of the Integrated Quality team and Interprofessional Practice
program, Managers, Directors, and patient care teams to identify learning needs for staff
members, teams, and the organization in relation to patient safety.
- Deliver presentations and training programs to increase awareness and understanding of
patient safety initiatives.
- Provide assistance and support to clinical programs and others in the development of
patient safety objectives, indicator selection/creation, monitoring tools, and target
